Job Description
Staff Physician Home Based Primary Care

The Home Based Primary Care (HBPC) staff physician practices in the outpatient setting in the Cheyenne VA Medical Center and in the residence of the client. Primary care includes assessment, diagnosis, treatment and outcome measures of healthcare problems. This includes the following: initiating and adjusting therapeutic regimens, performing histories and physicals, ordering diagnostic and therapeutic tests/studies, initiating consults and referrals, and prescribing medications/supplies in accordance with VA policy, VA formulary, Wyoming state regulations and scope of practice. Executes position responsibilities that demonstrate leadership, experience, and creative approaches to management of complex patient care.

In addition to providing primary health care, the MD serves as a consultant to Nursing and Medical Staff for patients with complex treatment or care needs in the home. Duties may include, but are not limited to:

-Provides medical care to patients with acute and chronic health problems
-Provides preventive medical care including screening and vaccinations
-Provides follow-up with additional home visits as appropriate
-Collaborates with nursing personnel in the care of patients
-Monitors the program to assure quality of care while meeting performance measures
-Performs some administrative and coordination duties of the interdisciplinary HBPC team

Work Schedule: Monday - Friday, 7:30am - 4pm

Basic Requirements:
  • United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
  • Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed.
  • Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia.
  • Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification.
  • Proficiency in spoken and written English.
  • Physical requirements outlined below.
Preferred Experience: Training and experience in Primary Care is strongly preferred. Prior experience in a Home Based Primary Care service would be beneficial.

Physical Requirements: You must pass a pre-employment physical. The incumbent must by physically able to drive to the client's home to provide holistic care and case management in the home of the veteran and will be based in Cheyenne, Wyoming. *The position requires that the incumbent must possess a valid driver's license. Eligible applicants must be physically and mentally able to perform efficiently the essential functions of the position, with or without reasonable accommodation, without hazard to themselves or others. Depending on the essential duties of a specific position, usable vision, color vision, hearing, or speech may be required. However, in most cases, a specific physical condition or impairment of a specific function may be compensated for by the satisfactory use of a prosthesis or mechanical aid.
