Job Description
Oncology Nurse Navigator - Green Hills

JOB CODE INFORMATION:

The Vanderbilt-Ingram System for Timely Access (VISTA) nurse is responsible for managing the new patient access to our Cancer Center. Specifically, each nurse is assigned to a group of physicians. For each new patient he/she works with administrative support to ensure the patient is scheduled with the correct specialist and then request outside medical records, pathology slides and radiology images. The admin person organizes the records for the nurse to review. The nurse reads the entire chart, identifies missing pieces and requests those. The nurse then calls the patient to confirm the medical, surgical, social and oncologic history and documents in the electronic medical record (EPIC). He/she confirms the patient is scheduled to see the correct provider, knows where to go and where to park, and answers any questions.

This is a small team of five nurses and four administrative staff. I am looking for a TEAM PLAYER who will help others when they are not busy. Without being asked!

Five days a week, 40 hours per week.
