Case Manager Position Title: Case Manager Join Our Mission to Empower and Transform Lives As the Case Manager, you would be responsible for providing services to enable individuals and families to obtain access to needed medical, social, educational, and other support services as well as providing counseling and therapeutic services as specified. The FIS function can be generally described as assessing, advocating, linking and coordinating needed community-based services for the client(s) and families served. The Case Manager will be heavily involved with and supportive of a clients educational, legal, vocational, extra-curricular and health related goals, with regular promotion of the accomplishment of these goals. The FIS generally meets with the client(s) and the clients family (foster, adoptive or biological) in the clients home or residence, on a regularly scheduled basis. In addition, the FIS may meet with the members of the clients community at locations such as school, daycare, job site, etc., as specified to meet the clients service needs. The Case Manager FISs hours of work will vary depending on the individual needs of his/her caseload but are generally able to be scheduled in a manner that does not exceed 40 hours in a given week. What you bring to this role: - Bring your passion and expertise - A desire to help others become the best they can be in serving our clients while managing a healthy work/life balance. - Ability to be a team member and thought leader. - Bachelors level degree (B.A. or B.S.) in the social sciences with (1) year relevant professional experience working with families and children. Although a degree in social sciences is preferred, a bachelors level degree in another field is permissible with (3) years of relevant professional experience working with families and children. - Training in proper crisis prevention initiatives and universal precautions required. - Proof of Automobile Insurance, Reliable Transportation & Valid drivers license.
