 • Facilitate outbound/inbound calls to recently discharged hospital patients. Facilitate referrals in the work queue within 48 hours. • Support SSM Health Physicians with allocation of Social Needs and Resources and Transition of Care. • Conducts psychosocial assessments/Social Determinants of Health to determine needs of patients and family members. • Coordinates treatment and communicates with multi-disciplinary teams to facilitate continuity of care for patients, and to improve standard of work procedures. Execute timely discharges. • Acts as a consultant to other team/staff members. Train other team members and staff. • Provides a full range of social work services within commonly accepted standards of social work practice which includes, case management and other services as appropriate. • Provides wellness/prevention education and facilitate patient/family support groups as appropriate. • Develops and maintains working relationships with community health, welfare, and social agencies and seeks creative means to assist the patient with his/her needs.
