 •Run and sort 1,000+ monthly reports for patient medications, nursing orders, referral communication, facility location information and regularly update pertinent information according to it’s fitting CRM (PointClickCare & Matrix). •Facilitate bi-weekly meeting agendas with each team manager based on team responsibilities. Each of the 4 teams carry unique, varying patient loads. •Service 100+ weekly calls as a direct point of contact for patients, family members and other facilities/physicians regarding patient care. •Successfully completed Relias compliance training include HIPAA compliance, complaint handling, basic wound care, infection prevention, etc. •Build schedules for in-patient and out-patient nurses and STNAs considering changes due to cancellations, deaths or changes in availability.
