Senior Case Manager for Homeless Women

Posted By: How May I Help You NC on August 27, 2012 Steadfast House – Case Manager for Single Women (Veterans & Civilian): Actively pursues and encourages volunteer and church participation in each of the program areas of responsibility.Utilize daily the Steadfast House HEART Program to connect community resources to each client that will provide self-esteem, stability, self-determination, spiritual formation, skill building, job placement, disability income, permanent housing or permanent supportive housing.Interview, screen, and conduct intakes and assessments of chronically homeless and homeless women to determine program eligibility. Provide case management to approximately 18 women (veteran & civilian) in a supportive and caring environment.Develop plans and address basic needs, housing, mental health, substance abuse, domestic violence and trauma issues in a coordinated way; drawing on both formal helping systems and natural supports, including spirituality Establish goals and strategies for increasing self-sufficiency through living wage employment or education tract. Assist residents with completing supportive documents such as identification, social security cards, food stamps, etc. Maintain client files; evaluate client progress to ensure program compliance. Maintain up to date resident status regarding discharges and vacancies at the Steadfast House. Responsible to carry out services to former veteran residents through our Follow-Up Program which offers: Furniture/Household Goods; Emergency Food, Problem-Solving. Advice/Resources. Holiday Help.
