Posted By: How May I Help You NC on November 07, 2012 Job Title: Clinical Pharmacist Care Transition - Part Time Location: Newark, DE General Description: PRIMARY FUNCTION: This is a part time position working 20 hours per week. Provide direct inpatient care activities by designing, recommending, monitoring and evaluating patient specific pharmacotherapy. PRINCIPLE DUTIES AND RESPONSIBILITIES: - Participate in rounds on panels of patients with multi-disciplinary care management team. Provide verbal and written consultations concerning drug therapy evaluations as requested. - Document drug use and compliance patterns, detect and evaluate adverse drug effects, evaluate drug efficacy, therapeutic goals and indications, review for contraindications/precautions, the patient’s current status of health, and review drug profiles and patient’s medical record for drug-drug, drug-food, drug-laboratory and drug-disease state(s) potentials. - Evaluate appropriate laboratory tests necessary to monitor and support the patient’s drug therapy program. - Document assessment, observations, impressions, recommendations, plan, significant findings, interventions and services rendered as appropriate in the care management system. - Coordinate activities as required with the care management team to perform enhanced medication reconciliation at admission, transfer and discharge on high-risk populations and patients or as indicated by assessment. - Report, record assess and evaluate adverse drug reactions to providers. - Provide accurate, unbiased, timely and cost-effective drug information to the patient and family at medication reconciliation. - Serve as a resource to suggest pharmacotherapy alternatives to providers to enhance compliance, reduce potential errors, adverse drug events and to minimize cost burden. ADDITIONAL DUTIES AND RESPONSIBILITIES: - Demonstrate skill and knowledge necessary to provide pharmaceutical care to adult and geriatric patients. - Works to integrate IT into workflow to enhance communication about medications across care settings. - Support and participate in other duties as required to meet workload demands. - Performs assigned work safely, adhering to established departmental safety rules and practices. Reports to supervisor, in a timely manner, any unsafe activities, conditions, hazards or safety violations that may cause injury to oneself, other employees, patients and visitors. SCOPE, PURPOSE, AND FREQUENCY OF CONTACTS: Frequent contact with physicians, nursing personnel, pharmacists, other providers in the community, and patients and their families. DIRECTION/SUPERVISION OF OTHERS: -Advisory function with the inpatient pharmacists and technical support personnel. DIRECTION/SUPERVISION RECEIVED: - Manager, Care Management Hub - Matrixes to VP, Pharmacy Services EDUCATION AND EXPERIENCE REQUIREMENTS: - Pharm.D. and ASHP-accredited Pharmacy Practice Residency required (or equivalent experience) - Current licensure (or eligible to reciprocate) in state of Delaware. KNOWLEDGE, SKILLS, AND ABILITIES REQUIREMENTS: - Knowledge of current pharmacy principles and practices - Knowledge of Christiana Care and departmental policies, procedures, and standards - Knowledge of federal and state laws governing pharmacy practices - Knowledge of JC requirements and standards - Knowledge of pharmacokinetic principles of therapeutic drug monitoring - Ability to apply knowledge to age and diagnosis specific patient situations - Ability to work independently within established policies and procedures - Ability to accomplish established program goals and objectives - Ability to exercise judgment, tact and diplomacy - Ability to handle fast-paced, stressful situations - Ability to practice principles of patient-centric service - Ability to maintain Christiana Care confidentiality standards - Skills in oral and written communications SPECIAL REQUIREMENTS: None PHYSICAL DEMANDS: Standing and walking with intermittent sitting. Requires movement throughout the hospitals of the Christiana Care. Repetitive hand movements required to operate PC keyboard. WORKING CONDITIONS: Generally fast-paced, potentially stressful environment with constant exposure to patient and families. Possible exposures to infections, communicable diseases, open wounds, chemicals, solvents, chemotherapeutic agents, and needles.
