Keep track of the health information of the person in your care. Choose just the pages you need, or download all of them to create a complete Caregiver Notebook.

The day-to-day
Use this form to jot down the details of daily routines, limitations, and needs of the person in your care.

Medicines
Make a detailed list of all prescriptions and over-the-counter medicines plus vitamins, supplements, and herbals the person in your care takes.

Personal health information
Record the personal and family medical history, current health **** information, and recent exam and **** results of the person in your care.



Health care contacts
Create a list of names and phone numbers of doctors. Don’t forget to include physical therapists, nutritionists, or other specialists the person in your care may see. If multiple people are managing his or her care, create a copy of the list for everyone to keep on hand.

Health care visits
Keep track of medical appointments and costs for the person in your care. Make a list of questions and updates to share with his or her doctors to help you stay organized.
