Day - 08 Hour (United States of America)The Advanced Practice Provider (APP) on the Inpatient Addiction Medicine Consult Team plays a critical role in delivering high-quality, evidence-based care to hospitalized patients with substance use disorders. This position requires advanced clinical expertise in addiction medicine, strong skills in assessment and management of withdrawal and co-occurring conditions, and the ability to collaborate effectively within a multidisciplinary hospital team. Responsibilities: 1. Inpatient Addiction Medicine Evaluation: Conduct comprehensive assessments of hospitalized patients with suspected or confirmed substance use disorders, including evaluation of withdrawal risk, substance use history, psychiatric and medical comorbidities, and readiness for treatment. 2. Treatment Planning and Implementation: Develop and implement individualized, evidence-based treatment plans for substance use disorders, including initiation and management of medications for addiction treatment (e.g., buprenorphine, methadone, naltrexone) and withdrawal management protocols. 3. Withdrawal Management and Stabilization: Assess and manage acute withdrawal syndromes (e.g., opioid, alcohol, benzodiazepine) using standardized protocols. Provide close monitoring and adjust treatment plans to ensure patient safety and comfort during hospitalization. 4. Medication Management: Initiate, titrate, and monitor medications for substance use disorders and co-occurring psychiatric conditions. Provide patient education on medication purpose, side effects, and adherence. 5. Consultation and Care Coordination: Collaborate closely with hospitalists, surgeons, psychiatrists, social workers, case managers, and nursing staff to provide integrated care. Serve as a consultant to primary teams for management of substance use disorders and related complications. 6. Discharge Planning and Continuity of Care: Facilitate transitions of care by coordinating referrals to outpatient addiction treatment programs, harm reduction services, and community resources. Ensure safe discharge planning, including access to medications and follow-up care. 7. Patient Education and Harm Reduction: Provide education on substance use disorders, overdose prevention, and harm reduction strategies (e.g., naloxone distribution). Engage patients in shared decision-making and support motivation for recovery. 8. Documentation and Compliance: Maintain accurate and timely documentation of consults, treatment plans, and care coordination. Ensure compliance with regulatory, legal, and institutional standards.This is a Stanford Health Care job.
