Pharmacy Technician - Medication History Program
The Medication History Technician plays a crucial role in patient admissions and medication reconciliation by obtaining and documenting accurate medication histories under the supervision of a licensed pharmacist for patients admitted to the hospital. This technician may also assist with other pharmacy technician duties.

In this specialized role, youll help ensure accurate medication histories are captured and documentedlaying the groundwork for effective treatment and reducing the risk of medication-related errors.

What Youll Do:

  • Gather and document complete medication histories for patients admitted to the hospital, under the supervision of a licensed pharmacist.
  • Support medication reconciliation efforts to improve patient safety and reduce adverse drug events.
  • Assist with other pharmacy technician responsibilities as needed, supporting overall pharmacy operations.
  • Contribute to patient progression by ensuring timely and accurate medication information is available to care teams.
  • Collaborate with pharmacists, nurses, and providers to ensure continuity of care and clear communication.

The successful incumbent in this position would be expected to work primarily evening shifts, and this position offers a schedule of 7 days on and 7 days off. Final scheduling accommodations will be determined by the hiring manager.

Minimum Qualifications

  • Education: High School Graduate or Equivalent
  • Experience: 6 months minimum relevant experience required. Preferred experience with patient admissions and medication reconciliation by obtaining and documenting accurate medication histories under the supervision of a licensed pharmacist for patients admitted to the hospital.
  • Licensure/Certifications: Commonwealth of Virginia Pharmacy Technician Registration required. CPhT credential through National Healthcareer Association (NHA) orPharmacy Technician Certification Board (PTCB) required. For team members hired prior to February 1, 2020, certification as CPhT through NHA or PTCB is required by January 31, 2022.

Physical Demands Job requires standing for prolonged periods. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull less than 20lbs.

Onboarding Requirements

Background checks and pre-employment health screenings will be conducted on all new hires prior to employment. All safety-sensitive positions with direct patient interaction also require a drug screen.

The starting base rate for this role is $20.68 hourly. Individual compensation will be determined by the selected candidate's qualifications, previous work experience, and/or education.
