<p><strong>JOB TITLE:                          </strong>Supportive Housing Specialist</p>
<p><strong>EMPLOYMENT TYPE:        </strong>Full-time, 40 hours/week</p>

<p><strong><span style="text-decoration: underline;">ABOUT THE POSITION:</span> Supportive Housing Specialist</strong></p>
<p><strong>Duties & Responsibilities </strong></p>
<ul>
<li>Provide targeted case management such as establishing rapport with clients, writing comprehensive treatment plans, provides counseling and support, providing direct skill teaching, advocacy and assistance in obtaining/maintaining benefits, budgeting, treatment, medical services, employment, transportation etc.</li>
<li>Maintain family and natural supports as well as other collateral contacts to ensure quality client- centered care with duties such as assessing and involving natural supports in the treatment process, networking with homeless service providers, liaison with landlords and provide overall case coordination.</li>
<li>Assists in understanding federal/state guidelines for rental subsidies, tenant rights responsibility as well as basic understanding of housing law as well as conduct outcome surveys as required</li>
<li>Maintain documentation in compliance with agency and program standards.</li>
<li>Participates in staff meetings, supervision, trainings and other meetings as assigned.</li>
<li>Additional duties as required.</li>

<p><span style="text-decoration: underline;"><strong>QUALIFICATIONS:</strong></span></p>
<p><strong>Education</strong><strong>:</strong>  Bachelor’s Degree in human services or equivalent education and experience </p>
<p><strong>Experience:</strong> 2 years experience working with the dually diagnosed (mentally ill, substance abuse)</p>
<p><strong>Licensure/Certification/Registration: </strong>Valid driver’s license.  CPR/First-Aid Certification within 6 months of hire desired.</p>

<p>$18.00/hr minimum, and up commensurate upon experience.</p>
<p>*Actual rates are determined at the time of offer and are based on relevant experience, as well as adherence to our internal equity policy. Many of our positions offer program/location differentials and/or sign-on incentives. (Please note, for part-time positions this range would be pro-rated based on hours worked)</p>
